Power of Attorney

I, the undersigned LAST NAME FIRST NAME, born in PLACE OF BIRTH (COUNTRY OF BIRTH) on DD/MM/YYYY, address of residence: ADDRESS, COUNTRY OF CITIZENSHIP citizen, passport number PASSPORT NUMBER, issued by: COUNTRY OF ISSUANCE, valid until DD/MM/YYYY, in the presence of a notary, hereby appoint attorneys dott. BOLLA NICOLO’ (codice fiscale BLLNCL90E07F463O) born in Montecchio Emilia on 07/05/1990 with office in Parma (PR), Piazza Gen. C.A. Dalla Chiesa n. 3, and authorize them to perform all actions jointly or separately, giving them the full right and power to perform all requested and necessary acts in order to request and obtain in my favor and on my behalf the clearance certificate, nulla osta, the certification of the work contract, visa and subsequent Residence Permit in Italy, as referred to in article 39 of the Presidential Decree of 31 August 1999 (Regulation containing the rules for the implementation of the Consolidation Act of the provisions concerning immigration regulations and norm on the condition of the foreigner) as amended by the D.P.R. 334 of 18 October 2004.
The aforementioned attorneys will be authorized to perform any action necessary and useful for the purpose of achieving the above mentioned purpose, including that of suing the competent authorities, both public and private, in relation to all the requirements and deeds and requirements necessary for the request and obtaining the relative permit, visa and subsequent Residence Permit for Italy in the name and on behalf of Mrs. LAST NAME FIRST NAME, giving them every right and full power to perform all procedures, in my absence, having the right to represent me before any authority and office in order to deposit and request any documentation as well as perform any other useful action that they deems appropriate and necessary, including the personal right to sign, giving consent to all procedures that have place in my absence and in the presence of the attorney appointed by me, with the possibility to perform all the operations mentioned above and other actions even if not expressly defined in this special power of attorney that must be carried out by the appointed attorney in order to realize my legitimate interests, performing all the actions that Italian procedural law recognizes to the represented person.

The represented Mrs. LAST NAME FIRST NAME now confirms the work of the aforementioned special attorney as valid and ratified as well as fully recognizes all the actions carried out in the name and on behalf of her by the appointed attorneys, being unable to do so personally.

This special power of attorney is drawn up according to the full will of Mrs. LAST NAME FIRST NAME, the text of which has been given full reading and corresponds to her will. Document accepted by her and signed in my presence, here, in front of me, a notary who certifies her signature according to the law of Italy.
